Your first visit
What happens on the first visit.
As part of your first visit you will fill out a comprehensive medical and dental history. These forms matter, because they tell us about allergies, sensitivity to anesthetics, long-term medications and anything else that should change what we do next.
- A comprehensive examination and a periodontal evaluation
- Necessary radiographs, which show tooth structure above and below the gum line

Dental insurance and payment
As a courtesy, if you have dental insurance we will send your claim directly to your insurance company. This can only be done if you provide us with accurate, up to date information, and we will notify you once we have received any payment from them.
Please note that your insurance policy is a contract between you and your insurance company. It is important that you check your benefits in your policy booklet. As there are thousands of different policies, it is impossible for us to tell you what procedures are covered by yours and to what extent.
Pre-treatment estimates
We can ask your insurance company for a pre-treatment estimate of benefits if we have appropriate radiographs. This is typically done when extensive treatment is planned, and it will delay treatment for up to eight weeks while we wait for the company’s response. A pre-treatment estimate contains several disclaimers and is not a binding promise to pay for any part of your treatment.
What is due, and when
While you are responsible for your entire bill, we will not ask for payment for amounts we estimate to be covered by your insurance. Any amount not estimated to be covered by insurance is due and payable at the time treatment is rendered.
Please ask us about other payment options when planned treatment exceeds $1,500, or if there are extenuating circumstances. We accept all major credit cards and can give you information about financing options.
Please do not submit any Protected Health Information through this website. The appointment form asks only for what a scheduling coordinator needs in order to call you back. Anything clinical is better said on the phone or in the chair.
